Use of Raltegravir to Prevent Perinatal Transmission

This case report illustrates the use of raltegravir for viral load supression late in pregnancy.  Although safety and pharmacokinetic data are not available for the use of raltegravir in pregnant women, it has been used in limited cases where the patient has multidrug resistance, late presentation, or poor adherence and requires an integrase inhibitor to lower viral load to decrease transmission risk.  This case report shows the use at 33 weeks gestation for quick reduction of viral load and can be used to further guide the use of raltegravir in this population.
