Background

Q: What is enhanced case management?
A: Enhanced case management is intensive case management. It is a field-based service where the case manager brings the services to the client. Enhanced case management is often targeted at especially vulnerable populations, like pregnant women, bringing the case management to their homes and their neighborhoods.

Q: How does enhanced case management differ from traditional case management?
A: Traditional case management usually occurs in the case manager’s office, requiring the client to make and keep appointments. Enhanced case management occurs in the field. Case loads for enhanced case managers are small. They usually have one-quarter to one-half of the number of clients that a traditional case manager has. In enhanced case management, the visits are longer and clients usually require more hands on assistance in the field such as escort to appointments, linkage to specialized medical care, outreach and home visits.

Q: Is enhanced case management provided only to HIV positive pregnant women?
A: Enhanced case management is provided to HIV positive pregnant women, recently delivered HIV positive women and women with a preliminary positive rapid HIV test on labor and delivery.

Referral for Services

Q: How do I get my client linked with enhanced case management?
A: By calling the Illinois Perinatal HIV Hotline you can request linkage with enhanced case management. You will be faxed a release of information form that the client must sign in order to be contacted by the case manager.

Q: Can I get enhanced case management for a client who is already linked to care?
A: Enhanced case management is designed specifically for women with extra needs. It is a field-based service intended for women who have difficulty linking or staying linked to care.

Services Offered

Q: What services are offered by enhanced case managers?
A: Case managers can assist clients with referrals to prenatal and postpartum medical care, food and housing assistance, mental health and substance abuse treatment, and transportation or other social service needs including meeting with family and partners.

Q: How long will the enhanced case manager follow the client?
A: The case managers follow the client throughout the pregnancy and up to six months postpartum. The client may terminate the services at any time.

Services for Pregnant Women with Positive Rapid HIV Tests

Q: What happens when the enhanced case manager comes to the hospital to see a patient with a preliminary positive rapid HIV test result?
A: The case manager will meet with the staff that called the Illinois Perinatal HIV Hotline to report the preliminary positive test result and inquire about any particular issues of concern for either staff or the client. The case manager will then request to meet with the client privately (away from family) to discuss options. With permission, the case manager will meet with the family with the goal of stabilizing the situation. Before leaving the hospital, the case manager will update staff on any plans and referrals that might be in process for the client.

Q: What happens to the enhanced case management relationship if the rapid HIV test is confirmed negative?
A: The case work is completed and the client is referred to the appropriate follow-up services for HIV negative women. The clients are assisted with finding a pediatrician, where to go for benefits (WIC, KidCare) and the case may be transferred to a DHS Family Case Manager where appropriate.

Confidentiality

Q: Does the case manager maintain the client’s confidentiality – even when other family members may not know about the preliminary positive or confirmed positive HIV status?
A: The enhanced case manager is VERY SENSITIVE to the confidential nature of the HIV diagnosis and the preliminary positive HIV diagnosis. The case manager is not allowed, by law, to disclose this information to anyone without the client’s specific permission.
